David Hoke, MD MBE
Lead Carte Physician
Dr. Hoke is a Board-Certified Emergency Physician, Bioethicist, and Physician Advisor at Good Samaritan Hospital. In this role, he serves as a liaison between the hospital administration and the medical staff, as well as between the hospital and insurance companies. He has participated in research committees at Children's Hospital of Pennsylvania, as well as chair of a multi-system Ethics Committee. He has developed Professional Education curricula including launching a residency in Charleston, SC. Clinically, he has worked in a multitude of environments, from rural critical access hospitals to trauma centers and elite concierge care groups. In his free time, he enjoys fishing and spending time with his wife and two daughters.

Ashley Alwood, MD
Lead Carte Phsycian
Dr. Alwood is a board-certified emergency physician with a diverse international career. She was born and raised in Seattle, Washington and spent part of her childhood living in London. After completing her medical education at the University of Washington and residency at the University of Chicago, where she also trained as a Flight Physician, Dr. Alwood practiced in Chicago before moving to the Middle East. She contributed to establishing Cleveland Clinic Abu Dhabi and later The View Hospital in Doha, Qatar, in affiliation with Cedars-Sinai Medical Center in Los Angeles, where she served as Head of the Emergency Department and a Clinical Assistant Professor.

Gina Siddiqui, MD
Lead Carte Physician
Dr. Siddiqui is the Founder and CEO of Carte Clinics. Prior to Carte Clinics, she was the Chief Clinical Officer of Fabric, an AI care enablement platform that scaled to cover over 100 million lives. During the COVID pandemic, she designed systems for medically complex patients at New York City Health and Hospitals. Dr. Siddiqui is on the Scientific Advisory Board for the Alliance for Longevity Initiatives and is Adjunct Faculty at the University of Virginia and George Washington University Schools of Medicine. She completed medical school at The University of Pennsylvania, residency training in Emergency Medicine at Yale, and dropped out of Harvard Business School.

Is there any obligation if I book the free call?

No obligation at all. We offer these consultations so you can experience how we think through your health challenges without risk. There’s no obligation to continue. You’ll leave the call with at least one actionable step you can take in the next 24 hours — guaranteed.

What if I don’t want to commit to an ongoing program?

That’s absolutely fine. The consultation stands on its own. Some people simply want clarity and a concrete next step — others decide to work with us longer-term. Either way, the decision is yours.

How is this different from a typical doctor’s appointment?

Most medical visits are rushed and narrowly focused. In this 30-minute call, we step back and look at the full picture: your parent's health history, lifestyle, and priorities. You’ll walk away with a personalized action plan, not just a prescription or referral.

Who will I be speaking with?

You’ll meet with one of our lead physicians above. We’ve guided hundreds of clients through complex medical decisions — and we bring that expertise to every consultation.

What if my situation is complicated? Will 30 minutes be enough?

Many of our clients come to us with complex histories — that’s our specialty. In 30 minutes we won’t solve everything, but we will give you clarity on the most important next step, plus a sense of how we can support you if you’d like deeper guidance.

Is this consultation or your service covered by insurance?

The consultation itself is free. Our ongoing work is not billed through insurance — that’s intentional. By staying outside the insurance system, we can serve as an independent advocate, focused only on what’s best for you. Many of our clients use insurance for procedures or tests, while we provide the strategy and coordination that insurance simply doesn’t cover.

Can I use HSA or FSA funds for this?

Yes. While we don’t bill insurance directly, many of our clients pay for memberships using HSA or FSA accounts. We’ll provide all the necessary documentation for reimbursement. We recommend confirming eligibility with your HSA/FSA provider before enrolling, as requirements can vary by plan.
